Informed Consent for Clinical Documentation Evaluation

Purpose of the Evaluation

This service provides a brief clinical evaluation to determine whether documentation supporting certain requests may be clinically appropriate. These requests may include pre-surgical psychological evaluations for adults seeking bariatric procedures.

The evaluation consists of completing an online questionnaire and participating in a brief structured clinical interview. These components assess mental health history, current symptoms, behavioral patterns, and relevant functional factors.

Nature of the Service

This service is not psychotherapy and does not establish an ongoing therapeutic relationship.

The purpose of this evaluation is limited to determining whether documentation supporting a request may be clinically appropriate at the time of the evaluation. The clinician does not provide ongoing treatment, formal diagnostic services for external records, or long-term mental health care through this service.

Limitations of Documentation

The evaluation reflects a point-in-time assessment based on self-reported information and clinical interview. Findings may change over time and do not represent an ongoing assessment of mental health status.

Third parties such as surgeons, medical staff, or other medical providers ultimately decide whether to grant requested surgical procedures. The clinician cannot guarantee that any procedure or request will be approved.

The clinician’s findings and recommendations are based on independent professional judgment and are not influenced by external parties or requested outcomes.

Communication with Third Parties

Calculated Practice does not communicate directly with surgeons, clinical staff, or other third parties. Documentation is provided solely to the client for their use.
