How It Works

Step 1 — Register for the Evaluation: Begin the checkout process, and be sure to enter an email that can receive the finalized PDF evaluation report.

Step 2 — Complete the Intake and Screening Forms: After payment, you will complete a structured questionnaire covering mental health symptoms, functional impairment, and relevant background information. Estimated time: 45–90 minutes (can be completed in multiple sittings). You'll also receive an email with a link if you need to resume the intake at a later time.

Step 3 — Clinical Interview and Review: Meet for a brief 20-minute structured interview. A licensed clinician will review your responses and determine whether surgery may be clinically appropriate.
